Dr. Mariano Arosemena is a board-certified vascular and endovascular surgeon at Vascular Wellness Institute in Sarasota and Bradenton, Florida. Fellowship-trained at Thomas Jefferson University and nationally recognized as a faculty proctor for TCAR — a leading minimally invasive stroke-prevention procedure — Dr. Arosemena brings advanced open and endovascular surgical expertise to the Gulf Coast, fully bilingual in English and Spanish.

A Nationally Recognized Specialist — Right Here on the Gulf Coast

When Dr. Arosemena relocated to Southwest Florida, patients here gained access to a level of vascular expertise that previously required traveling to a major academic medical center. Over his career, he served as Vascular Surgery Fellowship Site Director at the University of Maryland Baltimore Washington Medical Center and Clinical Assistant Professor of Surgery at the University of Maryland School of Medicine — roles in which he shaped the training of medical students, residents, and vascular surgery fellows.

Since 2018, Dr. Arosemena has served as a national consultant, faculty member, and physician proctor for TCAR (TransCarotid Artery Revascularization) through Silk Road Medical (now Boston Scientific), traveling across the United States to train vascular surgeons, interventional cardiologists, neurointerventionalists, and neurosurgeons in this advanced stroke-prevention technique. Very few vascular specialists in Florida hold this distinction.

In the office, Dr. Arosemena is known for his calm, methodical approach — taking the time to explain complex conditions clearly and involving patients and their families fully in every decision. Colleagues and patients alike describe him as thorough, unhurried, and genuinely invested in outcomes.

Areas of Clinical Expertise

TCAR — TransCarotid Artery Revascularization
TCAR is a minimally invasive stroke-prevention procedure that uses flow reversal technology to protect the brain during carotid stent placement. Dr. Arosemena is one of a small number of national faculty proctors for TCAR in the United States — meaning he trains other surgeons in this technique. For patients in Sarasota and Bradenton with carotid artery disease, this level of expertise is available locally.

Advanced Aortic Aneurysm Repair — EVAR & Fenestrated EVAR
During his fellowship at Thomas Jefferson University Hospital, Dr. Arosemena developed deep expertise in complex aortic disease, including fenestrated endovascular aneurysm repair (fEVAR) — an advanced procedure for aneurysms involving the renal or visceral arteries. fEVAR is not available at every vascular program; its availability in Sarasota and Bradenton reflects the academic caliber of care VWI is built to deliver.

Peripheral Artery Disease (PAD)
Dr. Arosemena offers the full range of endovascular PAD treatments — balloon angioplasty, drug-coated balloon (DCB) angioplasty, atherectomy, and stenting — as well as open lower extremity bypass for patients with advanced or limb-threatening disease. Treatment is always individualized to the patient’s anatomy, disease pattern, and overall clinical picture.

Thoracic Outlet Syndrome (TOS)
Dr. Arosemena specializes in both venous TOS (including Paget-Schroetter syndrome — arm DVT caused by vascular compression) and arterial TOS, providing surgical decompression via first rib resection. He coordinates with thoracic and neurological colleagues for multidisciplinary management of complex cases.

Venous Disease & Venous Stenting
From varicose veins and chronic venous insufficiency to complex deep venous obstruction from May-Thurner syndrome or post-thrombotic disease, Dr. Arosemena provides advanced evaluation and minimally invasive treatment for the full spectrum of venous conditions. In-office procedures are typically completed in under an hour with same-day recovery.

Diagnostic Expertise — RPVI Credential
As a Registered Physician in Vascular Interpretation (RPVI), Dr. Arosemena personally interprets duplex vascular ultrasound, ankle-brachial index (ABI) testing, venous reflux studies, and pre-operative vascular mapping. Direct physician interpretation means faster diagnosis and more precise treatment planning for every patient.

Bilingual Care — English & Spanish

Born and raised in Panama, Dr. Arosemena is fully fluent in English and Spanish and provides complete consultations, surgical discussions, and follow-up care in both languages — no interpreter required. For Spanish-speaking patients in Sarasota, Bradenton, and surrounding communities, this means receiving detailed explanations of a diagnosis and all treatment options in their primary language. In vascular medicine, where decisions often involve complex procedures and long-term management, that clarity matters enormously.

Southwest Florida’s large and growing Hispanic and Latino communities deserve the same access to specialist vascular care as any other patient population. Dr. Arosemena is committed to ensuring language is never a barrier to that care.

Pacientes hispanohablantes de Sarasota, Bradenton y Lakewood Ranch pueden consultar con el Dr. Arosemena en español para recibir atención vascular experta.

Dr. Arosemena’s Approach to Care

“I trained at institutions where the standard was always to do the most for the patient with the least risk. That means choosing the right procedure, not the easiest one — and taking the time to explain why. I want every patient who leaves my office to understand exactly what is happening in their body and exactly what we are going to do about it.”

Dr. Arosemena is known among colleagues and referring physicians for his methodical approach and the breadth of his procedural expertise. Patients consistently describe him as unhurried, thorough, and genuinely engaged — qualities that are easy to say and harder to find.

Frequently Asked Questions About Dr. Arosemena

  • Does Dr. Arosemena offer consultations in Spanish? +

Yes. Dr. Arosemena is fully fluent in Spanish and conducts complete consultations, surgical discussions, and follow-up care in Spanish for patients who prefer it. No interpreter is needed. Contact our office to schedule a Spanish-language appointment in Bradenton or Sarasota.

  • What is TCAR, and why does it matter that Dr. Arosemena is a national faculty proctor? +

TCAR (TransCarotid Artery Revascularization) is a minimally invasive stroke-prevention procedure that places a stent in the carotid artery using flow reversal technology to protect the brain during the procedure. As a national faculty proctor, Dr. Arosemena trains other surgeons and interventionalists across the United States in TCAR — a distinction held by very few vascular specialists nationally. For patients in Sarasota and Bradenton, this means access to one of the most experienced TCAR specialists available anywhere in the country.

  • What is fenestrated EVAR and is it available in Sarasota? +

Fenestrated EVAR (fEVAR) is an advanced endovascular technique for repairing complex aortic aneurysms that involve or are close to the renal and visceral arteries. It uses a custom-designed stent-graft with precisely positioned openings to maintain blood flow to the kidneys and abdominal organs while excluding the aneurysm. Dr. Arosemena performs fEVAR for appropriate patients — a procedure not available at all vascular programs.

  • What is the RPVI credential? +

RPVI stands for Registered Physician in Vascular Interpretation — a credential awarded to physicians who demonstrate specialist-level proficiency in interpreting vascular ultrasound studies. It means Dr. Arosemena personally reads and interprets the diagnostic studies that inform your care, rather than delegating that reading to a generalist.

  • Does Dr. Arosemena treat thoracic outlet syndrome? +

Yes. Dr. Arosemena specializes in vascular thoracic outlet syndrome — both venous TOS (Paget-Schroetter syndrome, which causes arm DVT and swelling) and arterial TOS. Treatment includes catheter-directed thrombolysis for acute clots and surgical decompression via first rib resection. He coordinates with thoracic and neurological colleagues for complex neurogenic TOS cases requiring multidisciplinary management.

  • What minimally invasive treatments does Dr. Arosemena offer for varicose veins? +

Dr. Arosemena offers several minimally invasive treatments for varicose veins and chronic venous insufficiency, including radiofrequency ablation (RFA), Varithena® microfoam sclerotherapy, and VenaSeal™. These procedures are performed in an outpatient setting under ultrasound guidance, typically allowing patients to return to normal activities the same day.

  • Is Dr. Arosemena accepting new patients in Sarasota and Bradenton? +

Yes. Dr. Arosemena is accepting new patients at both Vascular Wellness Institute locations — Sarasota and Bradenton. Same-week consultations are available for new patients. Physician referrals and self-referrals are both welcome.
