Leg Swelling

Leg swelling — also called lower extremity edema — is one of the most common reasons patients seek vascular evaluation. While many cases of mild ankle swelling are benign and positional, persistent, worsening, or asymmetric leg swelling often indicates significant vascular disease requiring diagnosis and treatment. Identifying whether swelling is venous, lymphatic, or systemic in origin determines the appropriate treatment pathway.

Vascular Causes of Leg Swelling

The most common vascular causes of leg swelling include chronic venous insufficiency and varicose veins — in which incompetent vein valves allow blood to pool in the lower extremity, driving fluid out of the veins into surrounding tissue; deep vein thrombosis (DVT) — clotting that obstructs venous outflow and causes rapid, severe unilateral swelling; May-Thurner syndrome — structural iliac vein compression causing left-leg predominant swelling; pelvic congestion syndrome — elevated pelvic venous pressure causing lower extremity and pelvic symptoms; and lymphedema — progressive non-pitting swelling from lymphatic obstruction.

When Is Leg Swelling an Emergency?

DVT carries a risk of pulmonary embolism (PE) — a blood clot in the lungs — which is a potentially life-threatening emergency. Treatment is directed at the underlying cause. Venous insufficiency and varicose veins are treated with endovenous ablation and sclerotherapy, combined with compression therapy.
