What Is Lower Extremity Bypass Surgery?
Lower extremity bypass is an open surgical procedure that creates a detour around a blocked or severely narrowed artery in the leg. A graft — either a segment of your own vein (usually the great saphenous vein) or a synthetic tube — is sewn above and below the blockage, creating a new channel for blood to flow.
The most common types include femoral-popliteal bypass (fem-pop), which bypasses blockages in the thigh, and femoral-tibial bypass, which extends further down the leg to the vessels near the ankle or foot. The specific bypass performed depends on where your blockage is located and the condition of the vessels above and below it.
When Is Bypass Surgery Recommended?
Your vascular surgeon will consider bypass surgery when:
- Peripheral artery disease (PAD) is causing severe symptoms such as rest pain or non-healing wounds
- Angioplasty or stenting is not feasible due to the length or location of the blockage
- A prior endovascular procedure has failed or re-narrowed
- Limb salvage is the goal — particularly in patients with diabetic foot wounds or gangrene
Vein vs. Synthetic Graft
When available, your own vein is the preferred bypass material because it is a living vessel that integrates into your body and resists infection. Vein bypasses — particularly when using the great saphenous vein — have superior long-term patency rates compared to synthetic grafts. If your vein is not suitable, a synthetic graft made of PTFE or Dacron is used instead. Your surgeon will assess your vein quality during pre-operative evaluation.
What to Expect
Lower extremity bypass is performed under general or regional anesthesia in a hospital setting. The procedure typically takes two to four hours depending on the complexity and the length of the bypass. Recovery involves a hospital stay of two to five days, followed by several weeks of limited activity at home. Most patients are walking with assistance before they leave the hospital.
Long-term graft monitoring with duplex ultrasound is important — regular surveillance allows early detection of graft narrowing before it progresses to failure.
